This form can be used by parents/guardians to report a student’s absence.  Parents/guardians may also use the "message" space to inquire about attendance matters or you can reach the attendance office directly by phoning 523-1623 or by email at JDHS_Attendance@jsd.k12.ak.us.  The attendance office will respond to all submitted forms to acknowledge their receipt.   Submissions using an email address not already on school records may result in a duplicate email to a listed address or a follow up phone call.   Thank you.

Note:  This absence notification form can be used to start the family-planned absence process.  Family-planned absences include any preplanned absence of one day or more such as family travel, club sport trips, or preplanned medical/dental procedures such as wisdom teeth extraction.  Please note the dates of the absence and the reason for the request in the comment section of this absence notification form.  Your student will then be given the goldenrod colored family-planned absence form to complete.  He or she will be required to get the signatures of all teachers before bringing the family-planned absence form home for your signature.  Please be sure to note your student’s grades and the assignments the teachers have listed on the form.  THE SIGNATURE OF A PARENT OR GUARDIAN IS REQUIRED ON THE COMPLETED FAMILY-PLANNED ABSENCE FORM BEFORE THE EXCUSE WILL BE ACCEPTED.  Please start the process a week to 10 days in advance of the absence so that your student will have time to complete the form in advance of the absence.   When possible, families should plan their extended travel during school holidays.
